このフォームからお申し込み後、タックス・インボイス＆申し込み内容が記載されたEmailが届きますので、1週間以内に年会費 $30をお支払いください。

• JCV会員（JCV member）：文化サロンにも参加でき、優遇されるイベントなどの情報メールが配信されます。

御家族情報の入力欄が足りない場合は、Notesに記入願います。

You will receive a confirmation e-mail including the tax invoice and your application details after submitting. Please pay the annual membership fee($30) within 1 week of receiving the e-mail.

If there are more than six in your family, please specify the details in the Notes field.

• 入金して申し込み詳細無し
• またJapan Festivalの出店のための入会はお断りしています

以上の方は入会をお断りしています 

• Applied, but payment not received
• Payment made, but no application details provided
• We do not accept membership applications solely for the purpose of participating in the Japan Festival
